The diet that's necessary to control the disease is so extreme as to create social ostracism: basically, eating any normal food is a medical problem, and the special diet involves food that is obviously medical diet food (rather than, say, the people being able to tell classmates or coworkers that they don't want to trade sandwiches, or brought a salad from home today).
