I have done some investigating re: Tony & heart surgery, because this bugged me.
The voiceover says that he "got Pepper sorted out" and then that he thought, "why stop there?" Then he says that yeah, there are "people who say progress is dangerous," but that none of them "ever had to live with a chest full of shrapnel."
So I think the reference to "progress" only makes sense as dosing himself with Extremis, and that he couldn't have done it before. But there is a psychological argument to be made about his state in _IM2_ (fatalistic, trying to go it alone) that would support that Extremis wasn't necessary if people disagree with my reading of that line. I guess we'll have to wait and see; apparently even though his contract is up he's widely expected to be in _Avengers 2_.
